The following outlines the benefits and responsibilities for the Holistic Family Wellness Care program.  By agreeing to these guidelines, we hope to provide a simple path for you to pursue holistic care.

Explanation of fees and benefits:

Initiation Fee: $100. This is intended to cover operational starting costs.

Membership Options:

  • Basic Monthly Membership Fee:
    • Adult: $85 ($950 if paid in full for year)
    • <18 y/o: $65 ($700 if paid in full for year) – one parent must also be a patient
       
  • Premium Monthly Membership Fee: (includes evening and weekend response)
    • Adult: $99 ($1,050 if paid in full for year)
    • <18 years old: $79 ($900 if paid in full for year) – one parent must also be a patient
    • Yearly Premium Membership gives you access to Hippocrates Evolution (HippEvo) Membership offering educational and integrative health tools. 
      • Includes 10% discount on supplements and programs.
      • Offers quarterly webinars for Q&A forums on a variety of topics.
         
  • Family Membership discounts are available for families enrolling more than 2 children (<18 years old).  See HippEvoShop.com for details.

What’s included:

  • 1 yearly 60-minute Wellness Discussion
    • This time will be utilized to discuss yearly health goals, lifestyle approach, and to develop a plan for optimizing health. 
    • Labs to assess metabolic status and overall health can be ordered and must be completed 3 weeks prior to your 60-minute Wellness Discussion to be reviewed at your appointment.
    • We recommend obtaining labs through LabCorp. These can be billed directly to your insurance at the time of the draw, or you can obtain discounted prepay pricing. Details can be found on the website and in the patient handbook.  Website: Huberpm.com > Patients > Labs.
       
  • 2 yearly 30-minute Osteopathic Manipulative Treatments (OMT)
    • OMT is a holistic treatment modality that utilizes hands-on therapy to assist in treating the body for general wellness or acute situations.
    • Additional OMT visits can be added through the year at a 10% discount from standard office rates. The discounted rate is $166.50 for a 30-minute visit.
       
  • Basic acute care visits. 
    • Should you require an acute or urgent intervention for such things as respiratory infections, injury, or unexpected illness then I am available to you at no additional service fee.
       
    • Basic Members are eligible to be seen by phone or office visit within 24 hours during standard office hours, Monday through Friday. Current in office availability is Tuesday, Thursday, and Friday.
       
    • Premium Members are offered acute care response during office hours (in person or phone) and after office hours (phone). 
      • Evening and weekend response through HIPPA compliant messaging system and telephone communication.
      • Message response is available until 9pm and will be answered before the end of the day. 
      • No overnight call accessibility.  Acute situations should be directed to Urgent Care or Emergency Department facilities. 
         
  • Availability subject to change during scheduled vacations.
     
  • The following tests and procedures are available as needed for in office appointments. (Pricing and availability subject to change)
    • Strep A rapid testing - $5
    • Urinalysis - $5
    • Urine pregnancy test - $5
    • Ear wax removal - $10
    • Simple biopsy and skin tag removal - $30
      • Price may vary for number of lesions engaged
    • Wart treatment – cryotherapy - $20
      • Price may vary for number of lesions engaged
    • Joint injection (steroid) - $30
    • Nebulizer treatment - $30
    • Additional testing and procedures will be added based on need
       

Additional office visits can be added at your discretion.  Members receive a 10% discount.

  • Integrative Medicine – reverse illness and reduce medication dependency
  • Osteopathic Manipulation Treatment (OMT)
  • Chronic disease/medication management visits
  • Office rates for services outside of those included in membership (10% discount)
    • 15-minute appointment rate is $100 – your cost would be $90
    • 30-minute appointment rate is $185 – your cost would be $166.50
    • 45-minute appointment rate is $285 – your cost would be $256.50
    • 60-minute appointment rate is $330 – your cost would be $297
  • Regenerative Joint Injections – pricing discussed after evaluation

Fees are collected yearly unless monthly installments are elected. These payments are made online using a credit card through HippEvoShop.com
